2008 Fairfield Day Camp Forms

Medical and camp forms: 

No child will be admitted to camp without a completed medical form documenting a physical examination conducted within two years prior to attendance at camp, signed by a physician.  All camp forms must be submitted to the YMCA office prior to the start of camp or your child will not be allowed to attend.  Forms can be downloaded from this page(below) or call the YMCA office and we will mail them to you.
